PATIENT INFORMATION

 

If you are a new patient to our office, you will be receiving a new patient packet in the mail prior to your first visit. Please review and fill out these enclosed forms:

• Patient Registration

• Medical History

A few other things to keep in mind as you prepare for your appointment:

• Bring your insurance card(s) with you to your appointment. (You may be required to pay a deposit prior to seeing the physician if you cannot present your insurance card at the time of your visit.)

• Secure a referral from your primary care doctor (if your insurance requires)

• Be prepared to pay your co-payment for the day?s services
